How to file a notice
Email [contact email] with: your contact details; identification of the copyrighted work; the URL(s) of the infringing content; a statement that you have a good-faith belief the use is unauthorised; a statement, under penalty of perjury, that your notice is accurate and you are the owner or authorised to act; and your signature.
Counter-notice
If your content was removed and you believe that was a mistake, you may send a counter-notice with the same contact details and a statement, under penalty of perjury, that you have a good-faith belief the content was removed in error.
Repeat infringers
Accounts that repeatedly infringe copyright will be terminated.
